Transfer of Care Hub (ToCH) opens at Queen Elizabeth Hospital

The opening of a new Transfer of Care Hub at Queen Elizabeth Hospital marks a significant shift in how patients in Greenwich experience the journey from hospital to home.

Bringing together NHS teams, social care, and voluntary sector organisations in one shared space, the hub is designed to improve how discharge decisions are made—particularly for people who need extra support after leaving hospital. By coordinating care in real time, the service aims to reduce delays, avoid unnecessary hospital stays, and ensure patients are discharged safely with the right support in place. 

 

For Greenwich residents, this development is especially relevant for those with complex needs, including older adults, people with disabilities, or those requiring rehabilitation or community-based care. Discharges involving multiple agencies—often the most challenging to organise—will now be managed through a single, joined-up system. 

 

The hub focuses on patients across Pathways 1–3, where additional support is required, while those able to return home independently will continue to be discharged directly by ward teams. A key feature is improved patient tracking, helping professionals respond more quickly to changing needs and availability of services. 

 

This approach reflects a wider NHS ambition to shift care closer to home, reducing pressure on hospitals while improving patient outcomes. For local people, it could mean a smoother, more coordinated transition out of hospital and better access to ongoing care in the community.
