Transfer of Care Hub (ToCH) opens at Queen Elizabeth Hospital
Bringing together NHS teams, social care, and voluntary sector organisations in one shared space, the hub is designed to improve how discharge decisions are made—particularly for people who need extra support after leaving hospital.
